APPLICATION FOR ADMISSION TO KINDERGARTEN, AY 2015-2016

Information and Guidelines For Parents

I. Some Important Considerations

A. The UPIS will admit only one hundred (100) students at Kindergarten for the academic year 2015-2016.
B. The UPIS as a service school of the University, has an admissions policy that mandates a 60-40 ratio of
students in favor of children of UP personnel who are either permanent, temporary or UP contractual upon
application.
C. The UPIS uses Filipino as medium of instruction from Kindergarten to Grade 4.

II. Qualifications for Admission

A. Applicants to Kindergarten must be 5 ½ to 6 ½ years old by June 2015.
B. Only those born within November 15, 2008 -December 31, 2009 are qualified to apply.
C. They should NOT have taken the UPIS Kindergarten Admission Test before.

III. General Instructions to Applicants/Parents 

A. Accomplish completely the application form and test permit form.

Be sure to include the following requirements:
1. Two (2) recent 1 ½” x 1 ½” identical photos pasted on designated box.
2. Original birth certificate from the National Statistics Office (NSO), plus a clear
photocopy. For those born abroad, include original passport, and a clear photocopy of
the passport.
3. A long white letter envelope with postal stamp for mailing of results. Write the names
of the applicant and his/her parent/guardian and address with zip code.

Additional requirements
4. For UP employees:
 a) 2015 Certification of Employment issued by HRDO
 b) For dependents who are adopted, proof of adoption

B. Procedure for Submission of Application Form

Step 1. Submit the accomplished application form, test permit form, and other requirements to the
faculty processors at the UPIS 7-12 Bldg (ramp area) on the date specified in the
application form.

Step 2. Pay Ph450 testing fee at the UP Cashier’s Office.

Step 3. Submit the official receipt of payment to the faculty processor who in turn will issue the
 applicant’s test permit.

Dependents of Non-UP Personnel ------------ February 03-11, 2015

Dependents of UP Personnel ------------------ February 12-13, 2015

Test Administration ---------------------------------- April 6-10, 13-15, 2015 Monday-Friday 7:00 – 11:00 (AM)

Release of results ------------------------------------ May 01, 2015 (Friday)

Deadline for filing intention to enroll --------------- May 08, 2015 (Friday)

The list of qualified examinees will be posted at the UPIS 7-12 Bldg., Grades 3-6 Bldg., K-2 Bldg.
and at the website www.upis.upd.edu.ph on May 1, 2015.

Letter of admission will be issued by the Principal’s Office on May 4-8, 2015.

Examinees who did not qualify will also be notified of the results by mail.
